LPN Wound Care Nurse

Responsibilities: - Provide wound care and treatment to patients with acute and chronic wounds - Assess patients' wounds and develop individualized care plans - Monitor and evaluate the progress of wound healing - Educate patients and their families on wound care techniques and prevention strategies - Collaborate with interdisciplinary healthcare team members to coordinate patient care - Conduct health coaching sessions to promote patient self-care and adherence to treatment plans - Assist with discharge planning for patients transitioning from hospital to home or other healthcare facilities Experience: - Minimum of 1 year of experience as a Wound Care Nurse - Proficiency in performing wound assessments, dressing changes, and wound debridement - Experience in utilizing advanced wound care techniques such as negative pressure wound therapy, hyperbaric oxygen therapy, and ultrasound therapy - Knowledge of specialized wound care products and their applications (e.
